**PLEASE REGISTER ALL CHILDREN UNDER ONE FAMILY ACCOUNT**

Physicals are submitted to the Main Health Office at BHS

Mon- Thur 8:30- 2:30pm

**FIRST TIME ATHLETES**

Your parent/guardian MUST create an online account. 

(keep this information on file)  

To register online:

Go to   bayonne-ar.rschooltoday.com

Click on the BLUE View My Account in the top right hand corner...

Register for the current sports season…. If you are interested in an out of season sport, register under Club/Activity and choose Weightroom.  This will let you create your account and participate in workouts (you must have a valid athletic physical).

**ATHLETES WHO HAVE REGISTERED PREVIOUSLY FOR A HS or ELEMENTARY SPORT**

Go to the login page. Click blue View my account. Log in under previous users. 

**PHYSICAL FORMS CAN BE DOWNLOADED FROM THE REGISTRATION PAGE OR PICKED UP OUTSIDE THE TRAINING ROOM!

**If you have food allergies or asthma, you must submit the necessary medical forms before you will be cleared to participate.  They can be downloaded off the online registration page** They must be completed by your private doctor

The Physical, may ONLY be completed by a NJ licensed physician MD/DO, advanced practice nurse (APN)or physician assistant (PA) that has completed the Student-Athlete Cardiac Assessment professional development module.

*** It is recommended that you verify that your medical provider has completed this module before scheduling an appointment for a PPE.

High Tech & County Prep Students

Enter elementary school ID number.

Enter HT/CP for homeroom number.

If you are attending HT/CP and did not go to a BBOED elementary school, you MUST register as a District Athlete in the attendance office.
