Physicals are submitted to the Main Health Office at BHS

Mon- Thur 8:30- 2:30pm

**FIRST TIME ATHLETES**

Your parent/guardian MUST create an online account. 

(keep this information on file)  

In the upper right hand corner, click View My Account. 

**PHYSICAL FORMS CAN BE DOWNLOADED FROM THE REGISTRATION PAGE OR PICKED UP OUTSIDE THE TRAINING ROOM!

**If you have food allergies or asthma, you must submit the necessary medical forms before you will be cleared to participate.  They can be downloaded off the available forms tab above.

High Tech & County Prep Students

If you are not currently attending Bayonne High School, you will first need to register as a District Athlete by completing an online application through our Registration Webpage .

You will need to provide a current report card or transcript indicating the name of your current school and your current grade.

Once your application has been approved, your student ID number will be provided to you and then you can proceed to register for athletics here.

Enter HT/CP for homeroom number.

The Physical, may ONLY be completed by a NJ licensed physician MD/DO, advanced practice nurse (APN)or physician assistant (PA) that has completed the Student-Athlete Cardiac Assessment professional development module.

*** It is recommended that you verify that your medical provider has completed this module before scheduling an appointment for a PPE.
